$16 per month

A hired individual who works for and is paid directly by Humana. $0 enrollment and $16 per month.  fitness gym, fitness gym.

COMMUNITY MEMBER

$45 per month

A community member (16 years or older). $45 per month. *No enrollment fee for former members. aaaaaaaaaaaa

HUMANA ASSOCIATE FAMILY

$32 per month

Associate + one or more individuals who live at the same residence (16 years or older). $0 enrollment fee and $32 per month.

COMMUNITY FAMILY

$75 per month

Community member + one or more individuals who live at the same residence (16 years or older). $75 per month. *No enrollment fee for former members.
